Medically necessary physician care is covered for eligible patients under the Alberta Health Care Insurance Plan (AHCIP). Coverage for medications and other services is separate.
What kinds of conditions does this cover?
Any ongoing health condition that needs regular monitoring and a coordinated plan — your family physician will discuss what that looks like for your specific situation.
Do I need to see a particular physician for chronic disease management?
No — every Blue Diamond family physician provides chronic-disease management as part of general family medicine. Book with whichever of our family physicians you are registered with.
How often will I need to be seen?
This depends entirely on your specific condition and care plan — your physician will set a monitoring schedule that's appropriate for you.
How do I book a chronic disease management visit?
If you are already registered with one of our family physicians, book with your doctor through Mikata. New patients and walk-ins can book online through Skip the Waiting Room, call the clinic, or come in — walk-ins are welcome.
